ASSESSMENT & SCREENING
Learn more about your balance, strength, mobility, and movement confidence through a practical screening designed with active aging in mind.
Elevate's Balance & Fall Prevention Screening uses a series of functional assessments to help identify strengths, areas to work on, and opportunities to support continued activity and independence.
Find a Screening What to Expect Explore Active Aging
Balance is not a single skill.
Strength, coordination, walking ability, reaction, mobility, and confidence all contribute to how we move through everyday life.
As we age, understanding these abilities can help us recognize changes, identify areas that may benefit from additional attention, and take practical steps to keep moving.
You do not need to have fallen or feel unsteady to benefit from learning more about how you move.
Get objective information about selected balance, strength, and mobility measures.
Recognize areas that may have changed before they become larger barriers to activity.
Identify movement, strength, balance, or activity areas you may want to work on.
Connect screening results with practical education, exercise ideas, and community resources.
This screening provides a snapshot of selected functional abilities.
It can help start a conversation about balance, mobility, strength, and fall prevention, but it is not a complete medical, neurologic, vestibular, or physical therapy evaluation.
Screening results should be considered alongside your health history, current activity, personal goals, and any symptoms or concerns you may have.
THE SCREENING
The screening combines several functional measures to look at different parts of balance, mobility, and physical function.
The exact combination may vary based on the event, setting, participant, and available time.
Looks at functional mobility as you stand from a chair, walk, turn, return, and sit down.
Provides information about lower-body strength and endurance through repeated sit-to-stand movement.
Explores standing balance through a progression of increasingly challenging foot positions.
Looks at dynamic balance, coordination, and the ability to step in multiple directions.
Provides information about functional walking and mobility over a measured distance.
Measures hand-grip strength as one additional marker of overall physical function.
Functional testing can help healthcare professionals look at different components of movement rather than relying only on how someone feels.
Measures such as the Timed Up & Go, 30-Second Chair Stand, and standing balance testing are commonly used when evaluating mobility, strength, balance, and factors related to fall risk in older adults.
No single score tells the whole story. Your results are most useful when considered together and discussed in the context of your individual abilities and goals.
Balance & Fall Prevention Screening is designed primarily for older adults and people focused on active aging.
You might consider a screening if you:
You feel well and want a better understanding of your current balance, strength, and mobility.
You have noticed that walking, stairs, getting out of a chair, or balance feels different than it used to.
You want practical information to support continued recreation, exercise, travel, and independent living.
You feel less steady, have become more cautious, or worry about losing your balance.
You have fallen and want to better understand factors related to balance and mobility.
You are working on strength, balance, or mobility and want objective information over time.
Screenings are typically led by a physical therapist, physical therapist assistant, athletic trainer, or other qualified healthcare professional.
We'll learn a little about your goals and any relevant mobility or balance concerns.
Complete a series of brief balance, strength, walking, and mobility activities.
Receive a score sheet and verbal explanation of your results.
Discuss education, exercise ideas, community resources, or possible follow-up when appropriate.
The goal is not simply to tell you how you performed.
Participants will typically receive a score sheet, verbal interpretation, education, and practical exercise recommendations based on the screening format and findings.
When appropriate, the screening professional may also suggest additional community programming or recommend discussing a concern with a healthcare provider.
Functional screening is intended to provide information, not a grade.
A lower score on one measure does not automatically mean that you will fall, and a strong score does not mean that falls cannot happen.
Your results can instead help identify areas of strength and opportunities to support continued movement and mobility.
ACTIVE AGING
Screening is most useful when the information leads to action.
Elevate's Active Aging Pathway brings together movement, strength, balance, mobility, education, recovery, and community programs for people who want to remain active throughout life.
Depending on your interests and screening results, you may want to explore classes, workshops, appointments, or other Active Aging opportunities.
Explore Active Aging Explore Classes
For some participants, a group movement class can be a practical next step.
Strong & Steady is one example of Elevate programming focused on strength, balance, mobility, and confidence for active aging.
The right program depends on your current abilities, goals, health, and any medical or activity restrictions.

A community screening is designed for general balance, mobility, and fall-prevention awareness.
It is not a substitute for a full evaluation when you have a new, significant, or unexplained medical concern.
A more comprehensive evaluation may be appropriate if you have repeated falls, a recent fall with injury, persistent dizziness, significant walking difficulty, or a noticeable decline in function.
The screening professional can help suggest an appropriate next step, but there is no required formal referral pathway from this program.
A scheduled balance screening is not the right setting for a sudden or potentially serious medical problem.
Seek prompt medical evaluation for new or rapidly worsening balance or walking difficulty, particularly when accompanied by symptoms such as new weakness or numbness, difficulty speaking, new confusion, fainting, severe or unusual headache, chest pain, shortness of breath, or other concerning symptoms.
Call 911 for symptoms that may represent a medical emergency.
Balance and mobility screening may be available as an individual appointment through Elevate.
For participants looking for a more individualized movement assessment, screening may also connect with Elevate's Ergonomics & Movement Analysis appointment offerings.
Current availability and pricing may vary.
