To improve the health of our community through promoting positive lifestyle choices, cultivating proactivity, resilience & growth, and empowering ambassadors to engage their own families and circles of impact.


Background

In 2019 Mammoth Hospital and Mono County Health Department conducted a Mono County Community Health Needs Assessment (CHNA) which engaged key stakeholders and community members to describe the health of local people, identify use of local health care services, identify community needs, and distinguish action needed to address the future delivery of health care in the region. The 2019 Mono County CHNA identified 4 health priorities including:

  • Substance Abuse Prevention and Treatment

  • Behavioral Health Prevention and Treatment

  • Clinical Care Access and Preventative Care

  • Dental Care Access and Preventative Care

In 2020 Elevate was established to address community health needs and improve population health through a collaborative, interdisciplinary, and community-based system with a lifestyle medicine focus. A lifestyle medicine approach to population care has the potential to arrest the continued rise in the prevalence of chronic conditions and their burdensome costs. Lifestyle medicine can address up to 80% of chronic disease and often results in better health outcomes, lower cost, improved patient satisfaction, improved provider well-being, and advancement of health equity and planetary health. Elevate is now Mammoth Hospital’s integrative lifestyle health and community wellness program which provides accessible resources toward addressing social determinants of health and root causes of chronic disease through evidence-based health education, activity classes, and social engagement. Elevate is contributing to the U.S. healthcare’s shift from a fee-for-service, “sick model” of reimbursement to a value and outcome-based system of healthcare delivery that promotes prevention of injury and disease, health maintenance, and optimal whole-person wellness.

Elevate operates from the theory that systemic change is most likely to occur when a network of community leaders and organizations align their goals and integrate their actions toward common results. Elevate collaborates with partnering individuals and organizations to provide community-based interventions and mutually reinforcing activities that promote health supportive behaviors for all.

    Health is for everyone! Our goal is to provide accessible and affordable services to support the health of all members of our community.

    We have programs in place to help you access the information, care, and support that you need. We offer recorded educational classes and some hybrid virtual-learning opportunities for those in our rural communities who are limited by schedule and transportation barriers.
